ServicesNursing Home Transitions When you are ready to go home… For patients who complete their rehabilitation therapies in the nursing home, referral to a home care agency will be made so that all needs for the transition to home can be met. This will be done at the time of discharge from the nursing home. If you need to go to the hospital… Patients can be sent to the hospital of their own choosing should the need arise. Patients sent to Holland Hospital, Zeeland Community Hospital or Spectrum Health Hospitals will be seen by a Michigan Medical, P.C. (mmpc®) Physician, if they have a Michigan Medical, P.C. primary care physician or by their own primary care physician. End of life For patients at the end of life, referral can be made to hospice and a hospice nurse will visit the nursing home. In special situations, patients at the end of life may be transferred to a Hospice House.
