Three Questions to Ask Your Insurance Provider

1. Do I have coverage for a medically supervised weight management program?

2. Do I have coverage for obesity?

3. What if my doctor recommends that I lose weight for health reasons?

 

Priority Health
You were given a pre-certification form at orientation.  This should be given to your primary care physician to be filled out and faxed in to Priority Health.  Your primary care physician will want to call Priority Health to get their criteria for the medical program.  Once we receive an authorization from Priority Health for you to attend the medical program, you appointments will be scheduled.

Participating Insurances
If the physician participates with your insurance, the office visits with the physician and the nurse’s only will be billed.  All other charges are non billable and payment will be expected on the day of your class.  If your insurance company denies the claim because office visits are not a covered benefit, or your only diagnosis is morbid obesity and that is not covered you will be responsible for the charge.  You will also be responsible for any co-pays, coinsurance and deductibles.

Non Participating Insurances  
We will expect payment on the day of service for all charges.  We will courtesy bill your office visit with the physician and the nurse’s only and payment from the insurance company will go directly to you.  All other charges are non billable and will not be courtesy billed.
